Enrolling a Child? Here's What You Will Need
We're so happy to have your child at Yoakum Primary!
If you are enrolling your child, please bring the following items.
- For the child
- Birth Certificate
- Social Security Card
- Immunization Records
- Records from the previous school, including
- Report card or withdrawal records
- STAAR and/or achievement test scores
- Information on Special Ed., 504, ESL, TELPAS G/T, At-Risk, Retention
- Legal Guardianship Papers
- For the enrolling person (must be a parent or legal guardian)
- Your valid driver's license or state-issued ID card
- Name, address and phone number for anyone who will be listed as an emergency contact for your child.
- Proof of residency, such as a utility bill
Please note that if you do not live within the boundaries of the Yoakum ISD school district, you will need to visit the Administration Office at 315 E. Gonzales St. to apply for approval for your student to enroll as a transfer student. Please call (361) 293-3162 for an appointment.
