Yoakum ISD Student Health Services     

Thank you for visiting the Yoakum ISD Student Health Services website.  The Yoakum ISD School Nurses are dedicated to promoting health and caring service by providing our students and staff with exceptional nursing care in an environment that empowers, educates, nurtures and guides them through their experiences in Yoakum ISD.  

Clinic Hours

Students are seen in the clinic during their school day by requesting a clinic form from their teacher if they are ill or injured.  If your child was seen during their school day, a copy of the clinic referral slip will be sent home with the student.  Please contact the school nurse if your emergency contact information changes during the school year.       

Students will be sent home if 

  • Their temperature is 100.0° F or higher
  • They are actively vomiting
  • They have diarrhea
  • They need to be seen by a physician
  • They have a contagious illness
  • The school nurse feels it is in their best interest

Helpful Information

  • Students may NOT return to school until fever of 100° (or greater), diarrhea and/or vomiting has stopped for a full 24 hours without the aid of medication.
  • Medications
    • Medications (prescription or over-the-counter) are NOT supplied by the school. 
    • Medication should NEVER be delivered by the child. 
    • Parents MUST bring medications to the clinic and fill out a Medication Administration Form (one form for each medication).
    • Yoakum ISD has a zero tolerance policy for drugs and alcohol; students may not have medication in their possession except with a physician's order and for emergency situations only. 
    • Medications MUST be in their original container. We request that you ask the pharmacist to give you two labeled prescription bottles so that you have one bottle at home and one at school. 
    • For inhalers, please ask the pharmacist to apply the label around the inhaler instead of the box of the inhaler. 
    • Please allow time for a pill count to be performed on all controlled medications. 
    • A new medication form will need to be completed and signed by the doctor for any medication changes.
  • A Doctor's signature is necessary on all Care Plans and Medication Forms (for medications that will be given for 5 or more consecutive days).
  • Food allergies of any kind are not recognized by our cafeteria staff unless we have a Food Allergy Action Plan signed by a doctor.  The forms are available from the school nurse.
  • If your child has seen a doctor and has started antibiotics, please keep the child home for 24 hours after the antibiotics have been started.

Texas Required Screenings

  • Hearing/Vision Screening is done in grades Pre-K, K, 1, 3, 5 and 7. Screening will also be done on any first time entrants in grades Pre-K - 12.  Please make sure if your child wears glasses or contacts that they bring them to school for screenings.  If your student did not pass the first screening, a second screening will be done by a nurse and you will be notified if the results require a referral for further care.
  • Acanthosis Nigricans Screening  is done in grades Pre-K, K, 1, 3, 5 and 7.
  • Spinal Screening is done either in grades 6 and 9 or 5 and 8.
